Total vs Free Testosterone: What Men Are Rarely Told

Most men are only shown one number: total testosterone.

That number rarely tells the full story.

Total testosterone measures how much hormone is present in the bloodstream. Free testosterone measures how much is actually available for the body to use.

Here’s the problem: testosterone binds to proteins—especially SHBG. When SHBG is elevated, free testosterone drops, even if total levels appear “normal.”

This is why many men are told their labs look fine while their symptoms continue.

Energy levels

Strength and muscle mass

Libido

Cognitive performance

Ignoring free testosterone leads to missed diagnoses and delayed care.
